Booking Info:

• Fill out a Group Payment Form (please write on the bottom of the form which cabin category you are interested in booking) then fax or scan and e-mail it to me before the deadline. Deposit of 20% of your total trip cost* is due no later then 2/24/10 - deposit is refundable prior to 4/1/10.

• Complete a Passenger Reservation Form and mail it back to me (sorry no faxes please) must be received by February 24th, 2010.

• Final payment is due 4/18/10. There is NO REFUND AFTER FINAL PAYMENT.

The above cruise rate does not include airfare, transfers to & from the ship or the registration fee.

Cruise Registration Fee:
A non-refundable registration fee which covers all of A Matter of the Heart seminars, special events, & activities on the cruise is $105.00 (check only).

The Early Bird Discounted Registration Fee will be due by 2/25/10. Regular priced Registration Fee is due by 3/1/10. ALL REGISTRATIONS FEES ARE NON-REFUNDABLE.

I will be out of the office from Monday, February 8th until Tuesday, February 22nd. You can still fax or e-mail your Group Payment Form in and I will book your cabin when I return.
